Medicare Part D: The Medicare prescription drug benefit program. We call this program “Part D” for short. Medicare Part D covers outpatient prescription drugs, vaccines, and some supplies not covered by Medicare Part A or Medicare Part B or Medical Assistance. Our plan includes Medicare Part D.Medicare Part A: The Medicare program that covers most medically necessary hospital, skilled nursing facility, home health, and hospice care.
Medicare Part B: The Medicare program that covers services (such as lab tests, surgeries, and doctor visits) and supplies (such as wheelchairs and walkers) that are medically necessary to treat a disease or condition. Medicare Part B also covers many preventive and screening services.
Medical Assistance: This is the name of Minnesota’s Medicaid program. Medical Assistance is run by the state and is paid for by the state and the federal government. It helps people with limited incomes and resources pay for long-term services and supports and medical costs.
It covers extra services and some drugs not covered by Medicare. Medicaid programs vary from state to state, but most health care costs are covered if you qualify for both Medicare and Medicaid.

Our plan will now cover one lab-created porcelain crown per calendar year.

+ Personal Emergency Response System Coverage

Qualified members can receive a MedScope personal emergency response system with no annual maximum.

AbilityCare covers:

  • Doctor visits including primary care and specialists, and other health care provider visits.
  • Hospital, urgent and emergency care.
  • Prescription drugs and durable medical equipment.
  • Home care including skilled nursing care, home health aide services and rehabilitation such as speech, physical and occupational therapies.
  • Vision, dental and chiropractic care. AbilityCare covers annual eye exams and new eyeglasses every 2 years. Members also have access to a broad dental network. Our plan covers one porcelain crown per calendar year. We cover chiropractic care and acupuncture.
  • Behavioral health. AbilityCare covers both inpatient and outpatient mental health and substance use disorder services, including treatment for schizophrenia, bipolar and major depressive disorders.
  • Transportation. RideConnect covers rides to and from medical, behavioral health and dental appointments for eligible members.

  • Preventive care rewards. Get gift card rewards up to $75 when you complete certain preventive care appointments.
  • Personalized care coordination. A registered nurse or social worker that can help you, your caregivers and your doctor manage health care services to meet your preferences and help you stay safe and healthy at home.
  • Better transitional care. AbilityCare does not have a mandatory 3-day hospital stay before entering a nursing home. A care coordinator can help you transition from one care setting to another.
  • A nationwide pharmacy network. We have nationwide pharmacy network with over 1,000 pharmacies in Minnesota.

SeniorCare Complete and AbilityCare are health plans that contract with both Medicare and the Minnesota Medical Assistance Program to provide benefits of both programs to enrollees. Enrollment in either plan depends on contract renewal.
