Medicare Part D: The Medicare prescription drug benefit program. We call this program “Part D” for short. Medicare Part D covers outpatient prescription drugs, vaccines, and some supplies not covered by Medicare Part A or Medicare Part B or Medical Assistance. Our plan includes Medicare Part D.Medicare Part A: The Medicare program that covers most medically necessary hospital, skilled nursing facility, home health, and hospice care.
Medicare Part B: The Medicare program that covers services (such as lab tests, surgeries, and doctor visits) and supplies (such as wheelchairs and walkers) that are medically necessary to treat a disease or condition. Medicare Part B also covers many preventive and screening services.
Medical Assistance: This is the name of Minnesota’s Medicaid program. Medical Assistance is run by the state and is paid for by the state and the federal government. It helps people with limited incomes and resources pay for long-term services and supports and medical costs.
It covers extra services and some drugs not covered by Medicare. Medicaid programs vary from state to state, but most health care costs are covered if you qualify for both Medicare and Medicaid.

How do I enroll in AbilityCare?

Steps to follow

1. Find out if you are eligible.

To be eligible for AbilityCare you must:

  • Be certified disabled by the Social Security Administration or State Medical Review Team (SMRT);
  • Be at least 18 or under the age of 65 at the time of enrollment;
  • Be eligible for Medical Assistance;
  • Have Medicare Parts A and B; and
  • Live in the South Country service area.

2. Learn more about the plan

Review the information on this website about AbilityCare. Member materials, such as the Member Handbook, are available to download in the Member Materials dropdown on the right. We will send you free printed copies of any member materials you ask for. Call Member Services for any benefit or coverage questions you may have or to ask for printed copies of member materials. You may speak with an enrollment specialist who can answer your questions about AbilityCare enrollment and verify if you meet the eligibility requirements. Free translation service is available if you need information in another language.

3. Fill out an enrollment form and send it to us

Enrolling in AbilityCare is voluntary, meaning you must fill out an enrollment form to join. If you have a printer, download and print the enrollment form found in the member materials section shown below. Send the form to us at our mailing address. If you prefer a form be mailed to you, please call Member Services and ask to have an enrollment kit sent to you.

More you should know...

Medical Assistance eligibility is determined by a financial/eligibility worker at your county human services department. You must be eligible for Medical Assistance to join AbilityCare. Contact your county human services department for more information about this process.

You do not pay a separate monthly plan premium for AbilityCare. If you pay a Medicare Part B premium, you must continue to pay your premium to remain eligible, unless your Part B premium is paid for you by Medical Assistance or another third party.

SeniorCare Complete and AbilityCare are health plans that contract with both Medicare and the Minnesota Medical Assistance Program to provide benefits of both programs to enrollees. Enrollment in either plan depends on contract renewal.
