Medicare Part D: The Medicare prescription drug benefit program. We call this program “Part D” for short. Medicare Part D covers outpatient prescription drugs, vaccines, and some supplies not covered by Medicare Part A or Medicare Part B or Medical Assistance. Our plan includes Medicare Part D.Medicare Part A: The Medicare program that covers most medically necessary hospital, skilled nursing facility, home health, and hospice care.
Medicare Part B: The Medicare program that covers services (such as lab tests, surgeries, and doctor visits) and supplies (such as wheelchairs and walkers) that are medically necessary to treat a disease or condition. Medicare Part B also covers many preventive and screening services.
Medical Assistance: This is the name of Minnesota’s Medicaid program. Medical Assistance is run by the state and is paid for by the state and the federal government. It helps people with limited incomes and resources pay for long-term services and supports and medical costs.
It covers extra services and some drugs not covered by Medicare. Medicaid programs vary from state to state, but most health care costs are covered if you qualify for both Medicare and Medicaid.

How do I enroll in SeniorCare Complete?

1. Find out if you are eligible.

To be eligible for SeniorCare Complete, you must:

  • Be at least 65 years old;
  • Be eligible for Medical Assistance;
  • Have Medicare Parts A and B; and
  • Live in one of the following Minnesota counties: Brown, Dodge, Goodhue, Sibley, Steele, Wabasha or Waseca.

2. Learn more about the plan.

Review the information found on this website about SeniorCare Complete. Member materials, such as the Member Handbook, are available to download in the member materials dropdown on the right. We will send you free printed copies of any member materials you ask for. Call South Country Member Services. for any benefit or coverage questions you may have or to ask for printed copies of member materials. Speak with an enrollment specialist who can help answer your questions about SeniorCare Complete enrollment and verify whether or not you meet the eligibility requirements. Free translation service is available if you need help in another language.

3. Fill out an enrollment form and send it to us.

Enrolling in SeniorCare Complete is voluntary, meaning you must fill out an enrollment form to join. If you have a printer, you may download and print the enrollment form found in the member materials on the right. Send the form to us at our mailing address. If you prefer to have a form mailed to you, please call South Country Member Services and ask to have an enrollment kit sent to you.

More you should know...

SeniorCare Complete eligibility is determined by a eligibility worker at your county human services department. You must be eligible for Medical Assistance to join SeniorCare Complete. Contact your county human services department for more information about this process.

You do not pay a separate monthly plan premium for SeniorCare Complete. If you pay a Medicare Part B premium, you must continue to pay your premium to remain eligible, unless your Part B premium is paid for you by Medical Assistance or another third party.

SeniorCare Complete and AbilityCare are health plans that contract with both Medicare and the Minnesota Medical Assistance Program to provide benefits of both programs to enrollees. Enrollment in either plan depends on contract renewal.
