Apr. 28, 2010 Primary Care Clinic Accessibility Survey
Click here for the web-based survey that will be open starting May 1, 2010 until May 31, 2010. Minnesota health plans that offer a Special Needs Basic Care (SNBC) product need to complete an access and accessibility survey of Primary Care Clinics for members with special health needs. Six Minnesota health plans have worked together to develop a collaborative survey to reduce duplication and multiple surveys to your Primary Care Clinics. Each Primary Care Clinic will ONLY need to complete the survey ONCE.

Mar. 31 , 2009 SCHA is providing the following disease management programs for members: heart condition, diabetes, and asthma. The programs are designed to reinforce your treatment plan and help members understand and manage their condition. You will receive a letter from SCHA when your patient has been identified as being eligible for the programs. These members are identified through medical claims as well as self or provider referrals. Members can be referred by contacting the disease management coordinators at:
Sept. 4, 2007 Effective immediately, SCHA will require submission of the Voluntary Sterilization consent form along with the sterilization claim. Claims submitted without the consent form will be denied back to the provider to submit the form. Find the forms on the Provider Forms page.
Sept. 4, 2007 SCHA is able to accept NPI#s in place of Provider MAID#s ONLY if the provider has registered the NPI# with DHS. See the following DHS web page for additional information here.
Aug. 1, 2007 MMSI Health Services will no longer process notifications or prior authorization requests by phone. Please utilize the notification/authorization request forms found on MMSI's web site at www.mmsiservices.com.

May 10, 2007 SCHA Authorization Requirements for CTSS and ARMHS. South Country Health Alliance has changed the authorization requirements for Children’s Therapeutic Support Services (CTSS). This change requires providers to notify MMSI prior to the start of services. MMSI will need the member’s name, identification number, initial date of service, and diagnosis. Providers will be given a notification number at that time for the initial 30 days of service.
The requirement to obtain prior authorization following the initial 30 days remains the same. The same authorization requirements will apply to the Adult Rehabilitation Mental Health Services, (ARMHS) which will become a covered benefit July 1, 2007, for South Country Health Alliance.
