
All members with a diagnosis of heart failure are automatically enrolled in the disease management program. They receive 2 welcome letters and a heart failure booklet. A phone call is placed to engage the member in the program. If a member chooses not to receive the phone calls, we offer the option of our mail only program. We work with the member by supporting their knowledge and compliance to the plan of care that they have with their doctor, specialists, and educators.
Disease Management does not take the place of existing medical providers or educators, but assesses the member’s knowledge of their health and supports the established plan of care. We review standards of care and good health care guidelines such as recommended by the American Heart Association, American Diabetes Association, and National Heart, Lung and Blood Institute. Member letters and educational materials are sent to DHS for approval prior to being sent to the member.
Our goal is to work together for the optimum health of our members. We also follow NCQA guidelines.
