To Owatonna: From the North

1. From I-35, take exit #42 (County Rd. 45) Merge right onto Hoffman Dr.
2. At the 3rd set of stoplights, take a left onto W. Rose St.
3. Turn left onto N. Oak Ave.
4. Turn right onto W. Fremont St. and go halfway down the block.
5. Enter the parking lot to your left across from the brick apartment building.

To Owatonna: From the South

1. From I-35, take the Bridge Street exit and turn right onto Bridge St.
2. At the 3rd set of stoplights, take a left onto Oak Ave. (which becomes Hoffman Dr.).
3. At the 2nd set of stoplights, take a right onto W. Rose St.
4. At the next intersection, turn left back onto N. Oak Ave.
5. Turn right onto W. Fremont St. and go halfway down the block.
6. Enter the parking lot to your left across from the brick apartment building.

To Owatonna: From the East

1. From Hwy 14 East, take the County Rd. 45 ramp toward Owatonna.
2. Turn right onto S. Cedar Ave/CR-45 N.
3. Shortly after the 2nd set of stoplights, merge left onto Oak Ave. and stay in the right lane as you travel all the way through the downtown district (about 2 miles) – Oak eventually becomes Hoffman Drive.
4. At the stoplights – corner of Hoffman Drive and Rose St., take a right onto Rose St.
5. At the next intersection, turn left back onto N. Oak Ave.
6. Turn right onto W. Fremont St. and go halfway down the block.
7. Enter the parking lot to your left across from the brick apartment building. Enter the SW door into our building.

To Owatonna: From the West

1. Take Hwy 14 E. into Owatonna (Hwy 14 eventually becomes Hoffman Drive).
2. At the 4th set of stoplights (Hoffman Dr. and Rose St.), turn left onto W. Rose St. At the first intersection, turn left onto N. Oak Ave.
3. At the first intersection, turn right onto W. Fremont St. and go halfway down the block.
4. Enter the parking lot to your left across from the brick apartment building.
5. Enter the SW door into our building.

• The Brainerd office is located at 322 Laurel Street, Suite 31 in the lower level of the Crow Wing County Land Services building.

To Brainerd: From the North

1. Follow MN-371 S to Baxter, MN.
2. Take a left onto MN-210 E / MN-371 S to Brainerd.
3. At the 4th stoplight, take a right onto S. 4th St.
4. The visitor parking lot is located two blocks on the right side of the street.
5. South Country Health Alliance is located on the south side of Laurel St. behind the Historical Courthouse and Museum in the Crow Wing County Land Services Building.
6. After entering the Land Services Building, take the elevator to the basement level. Take a left to suite #31.

To Brainerd: From the South

1. From US-10 W toward Little Falls, merge onto MN-371 N toward Brainerd.
2. Turn right on MN-371 Business District.
3. At the 4th stoplight, take a left onto Laurel St.
4. At the 2nd stop sign, take a right. Visitor parking is located immediately on the left side of the street.
5. South Country Health Alliance is located on the south side of Laurel St. behind the Historical Courthouse and Museum in the Crow Wing County Land Services Building.
6. After entering the Land Services Building, take the elevator to the basement level. Take a left to suite #31.

To Brainerd: From the East

1. Follow MN-210 W to Brainerd.
2. Take a left onto MN-371 S.
3. At the stoplight, take a right onto Laurel St.
