Disease Management & Chronic Care Improvement Programs

South Country Health Alliance provides a support service called Disease Management & Chronic Care Improvement Programs. This service helps our members who suffer from the chronic diseases of heart failure, diabetes or asthma. Disease Management empowers members to successfully deal with their chronic conditions and prevent complications. Our team provides one-on-one support, disease-specific education, and more. The service is entirely voluntary, and is provided at no cost.
