A Community Care Connector member is located in each of the South Country Health Alliance member counties. The Connectors help members get the services and supports they need. They work collaboratively with SCHA to provide support at the local level to meet SCHA’s mission: “Providing consumer-focused, community-based health care to Minnesota counties.”
