South Country Health Alliance was formed in 2001 to improve the way people receive health care. SCHA is now serving over 27,000 members i n 14 Minnesota counties (see map). We offer six different programs to meet your needs.

SCHA Health Promotions Programs and Incentives

SCHA offers a variety of health promotions programs and incentives that are designed to help remind and encourage you to take good care of your health. Learn why it's important to have a child's blood tested for lead, and when your child needs to have immunizations.

How do you determine whether a facility is accessible to people with disabilities?

If you require special access to obtain services from a provider, you can obtain more information by calling Member Services at 1-866-567-7242 or TTY 1-877-824-5611. Calls to these numbers are free. This could include access for services (but not limited to): Availability of flexible hours; wheelchair access; or parking lot access.
