Learn about our Health Promotion Programs
SCHA Offers These Programs:
- AbilityCare is a Special Needs Basic Care program designed for people with disabilities who live in our 12-county service area and are eligible for Medical Assistance and/or Medicare.
- SeniorCare Complete is a Minnesota Senior Health Options program for those age 65 or older that live in our 12-county service area and are eligible for Medical Assistance and Medicare Parts A and B.
- Minnesota Senior Care Plus is a program for those age 65 or older that live in our 12-county service area and are eligible for Medical Assistance.
- Medical Assistance (PMAP) is a health care program for families and children that live in our 12-county service area..
- MinnesotaCare is for those age birth and up, that live in our 12-county service area. There is a premium for each month of enrollment that is based on income, and determined by the state.
