South Country Health Alliance is dedicated to improving the health and well-being of government program recipients in out-state Minnesota. SCHA offers a full range of comprehensive benefits.

  1. Download the SCHA Employment Application (.doc).
  2. Open the file in Word. Type your responses in the appropriate fields, filling out the application carefully and completely.
  3. As you save, RENAME the file to include your last name, i.e., application_scha_JONES.pdf.
  4. Follow instructions for electronic submission (see below).
  5. Note: You may also choose to print and complete the application by hand. Please print carefully using blue or black ink. Scan and submit electronically, or follow procedure for submitting by mail (see below).

Electronically Submitting an Application

SCHA prefers applications to be submitted electronically. Attach your completed application, cover letter and resume to the email and send by the submission deadline to the person indicated in the job announcement. Note: All three documents must be received in our office by the posted deadline to be considered. Those missing an application form or submitted after the posted deadline will not be considered.

Submitting an Application by Mail or Fax (Acceptable)

If for some reason you are unable to submit your application electronically, you may instead mail or fax your completed application, cover letter and resume to the attention of the person indicated in the job announcement. Note: Submitting your application by mail or fax will not affect your application’s review by SCHA’s hiring committee, but submission deadlines still apply (i.e., received by deadline, not postmarked by deadline.) All three documents must be received in our office by the posted deadline to be considered. Those missing an application form or submitted after the posted deadline will not be considered.
