South Country Health Alliance (SCHA), a county-based health plan serving 12 Counties in Minnesota, has the following openings:

  • CLINICAL FACILITATOR: Responsible for development and implementation of health management programs for enrollees; develops tools and implements initiatives for monitoring, analysis and improvement of clinical and service outcomes; ensures contract compliance through implementation and oversight of delegated agencies and organizational work plans.

    Required: Current and valid, unrestricted RN license in MN or bachelor’s degree in Social Work or Behavioral Science; A minimum of 3 years clinical experience with at least 2 years of that experience working in public health, human services, long-term care, or other settings that cater to vulnerable populations.

    Minimum salary - $54,795/annual (Exempt)

  • CARE MANAGER: Responsible for completing health assessments in a timely manner, developing and implementing care plans for enrollees, and assisting enrollees in achieving their health goals; Acts as the care coordinator working with an interdisciplinary care team to manage the care and pharmacotherapy of enrollees.

    Required: Current, Valid and unrestricted RN License in Minnesota or Bachelor’s degree in Social Work or Behavioral Science, plus a minimum of 3 years clinical experience with at least 2 years of that experience in public health, long term care, or other settings that cater to vulnerable adult populations, such as experience working directly with the programs that serve frail members and/or vulnerable adults.

    Minimum salary - $50,736/annual (Exempt)

  • DISEASE MANAGEMENT COORDINATOR: Provides education to eligible enrollees (via telephone and U.S. Mail), who have chronic medical conditions for which SCHA has DM programs such as: heart failure, diabetes, and asthma in order to help improve their health status and reduce health care costs.

    Required: Current and valid, unrestricted RN license in MN; At least 2 years' experience in a hospital, acute care or direct care setting. County Public Health (PH) or Human Service (HS) experience meets above requirements.

  • SENIOR MANAGER - CLINICAL PROGRAMS: Leads/directs clinical program teams, delegates duties to staff, and responds to clinical operational issues/concerns while promoting quality and productivity in the administration of the clinical programs area of Health Services; accountable for meeting the financial, operational and quality objectives of the unit.

    Required: Current and valid, unrestricted RN license in MN; 5+ years relevant clinical experience; Medical knowledge of Medicare and/or Public Programs; International Classification of Diseases (ICD) knowledge and Current Procedural Terminology (CPT) coding experience.

    Minimum salary - $69,666/annual (Exempt)
